Duo-Jet® II Jet-Flow Repair Kit IMPORTANT RISK INFORMATION Duo-Jet® II Indications for Use: The Duo-Jet® II Catheter is designed for long-term hemodialysis and apheresis. It may be inserted percutaneously and is ideally placed in the internal jugular vein. Although this catheter may be inserted into the subclavian vein, the internal jugular vein of an adult patient is the preferred site. Contraindications: This catheter is intended for Long-Term vascular access only and should not be used for any purpose other than indicated in these instructions. To maintain peak performance, it is recommended that this catheter be replaced after 18 months usage. To maintain peak performance of the Lock right Adaptors, it is recommended that the adaptors be replaced every 6 months. Jet-Flow Indications for Use: The Jet-Flow Catheter is indicated for use in attaining Long-Term vascular access for Hemodialysis and Apheresis. It may be inserted percutaneously and is primarily placed in the internal jugular vein. Alternate insertion sites include the subclavian vein. Catheters greater than 40cm are intended for femoral vein insertion. Contraindications: This catheter is intended for Long-Term vascular access only and should not be used for any purpose other than indicated in these instructions. Jet Medical Repair Kit Indications for Use: The Jet Medical Repair Kit is indicated for use in replacing damaged female luer connectors, clamps or repairing extensions where there is a minimum of 4.5cm viable extension tubing. Repairs Jet Medical Catheters: Jet-Flow. Contraindications: Do not use to repair catheters other than those specified above. Do not replace connector if tubing is swollen or displays signs of degradation. This repair kit should not be used for any purpose other than indicated in these instructions. Refer to Instructions for Use provided with the product for complete instructions, warnings, precautions, and contraindications. Observe all instructions for use prior to using products. IMPORTANT RISK INFORMATION Duo-Jet® Indications for Use: The Duo-Jet® Catheter is indicated for use in attaining Short-Term vascular access for Hemodialysis and Apheresis. It may be inserted percutaneously and is primarily placed in the internal jugular vein of an adult patient. Alternate insertion sites include subclavian vein or femoral vein as required. The curved Duo-Jet® Catheter is intended for internal jugular vein insertion. This catheter is indicated for a duration less than (30) days. For femoral placement, monitor catheter condition closely. CONTRAINDICATIONS: This catheter is intended for Short-Term vascular access only and should not be used for any purpose other than indicated in these instructions. Duo-Jet® Soft-Line® Indications for Use: The Duo-Jet® Soft-Line® is designed for acute hemodialysis and apheresis. It may be inserted percutaneously and is ideally placed in the jugular vein. Contraindications: This catheter is not intended for any use other than that which is indicated. Do not implant catheter in thrombosed vessels. Jet-Cath® Side x Side Indications for Use: The Jet Cath® Side X Side double lumen catheter is intended for short-term central venous access for hemodialysis, apheresis and infusion. Contraindications: The catheter is not intended for any purpose other than indicated in these instructions. The Jet Cath® Side X Side double lumen catheter is intended for short-term (less than 30 days) use. Do not use this catheter in thrombosed vessels or for subclavian puncture when a ventilator is in use. Do not use this catheter when: The patient’s body size is insufficient to accommodate the implanted device. The superficial or deep tissue will not permit adequate device stabilization and/or access. There are known physiological limitations that will not allow placement of the device. The patient has known or suspected allergies to any of the materials in the device. The patient has received significant radiation at the intended exit site or tunnel. The patient has severe chronic obstructive lung disease. Jet Tri-Flow Indications for Use: The Jet 12F Tri-Flow Triple Lumen Catheter is indicated for use in attaining Short-Term vascular access for Hemodialysis and Apheresis. Jet I-Series Jet V-Series Jet X-Series PD Accessories IMPORTANT RISK INFORMATION Jet I-Series Indications for Use: The Peritoneal Catheter is indicated for chronic peritoneal dialysis. Contraindications: This catheter is contraindicated in patients who are not suitable candidates for peritoneal dialysis therapy. Jet V-Series Indications for Use: The Peritoneal Catheter is indicated for chronic peritoneal dialysis. Contraindications: This catheter is contraindicated in patients who are not suitable candidates for peritoneal dialysis therapy. Jet X-Series Indications for Use: The Peritoneal Catheter is indicated for chronic peritoneal dialysis. The extended length of the X-Series Catheter makes it especially applicable for peritoneal dialysis patients when it is necessary to locate the skin exit site remote from the usual lower abdominal region. The catheter may be particularly indicated in patients with obesity, floppy abdominal skin folds, urinary or fecal incontinence, chronic yeast intertrigo, intestinal stomas, or in patients who desire to take deep tub baths. Contraindications: This catheter is contraindicated in patients who are not suitable candidates for peritoneal dialysis therapy. An upper chest (presternal) exitsite location of the X-Series Catheter may be contraindicated in patients who have had breast augmentation or breast reconstruction or have a tracheostomy. However, this group of patients may be suitable candidates for an upper abdominal exit-site location of the X-Series Catheter. Barbed Luer Lock Adaptor Indications for Use: The Jet Barbed Luer Lock Adaptor is indicated for long-term use as a connection point for peritoneal dialysis catheters and male luers. Contraindications: Use only with catheter provided by Jet Medical S.A. Known or suspected allergies to the device. PD Catheter Stylet Indications for Use: The Jet Peritoneal Catheter Stylet is indicated for transient use to keep a peritoneal dialysis catheter straight and maneuverable during insertion into the peritoneum. Contraindications: Use only with catheter provided by Jet Medical S.A. PD Catheter Tunneler Indications for Use: The Jet Peritoneal Catheter Tunneler is indicated for transient use to dissect tissue to create the subcutaneous tunnel opening. IMPORTANT RISK INFORMATION Jet-PICC CT Indications for Use: The Jet-PICC CT Power Injectable Peripherally Inserted Central Catheter is indicated for short term or long term peripheral access to the central venous system for intravenous therapy and power injection of contrast media, and allows for central venous pressure monitoring, when a 20 gauge or larger lumen is used. For blood sampling, infusion, or therapies, use a 4F or larger catheter. The maximum recommended infusion rate varies by catheter French size and is printed on the catheter. Contraindications: The presence of device related infection, bacteremia, or septicemia is known or suspected. The patient’s body size is insufficient to accommodate the size of the implanted device. The patient is known or is suspected to be allergic to materials contained in the device. There has been past irradiation of prospective insertion site. There have been previous episodes of venous thrombosis or vascular surgical procedures at the prospective placement site. There are local tissue factors that may prevent proper device stabilization and/or access. Jet-PICC CT Jet-PICC Jet Port Jet LT CVC Jet Port Indications for Use: The Jet Port is indicated for patient therapies requiring repeated access to the vascular system. The port system can be used for infusion of medications, I.V. fluids, parenteral nutrition solutions, blood products and for the withdrawal of blood samples. When used with a power injectable needle, the power injectable infusion port is indicated for power injection of contrast media. For power injection of contrast media, the maximum recommended infusion rate is 5 ml/s with a 19- or 20-gauge non-coring power injectable needle. The maximum recommended infusion rate is 2 ml/s with a 22-gauge non-coring power injectable needle. Contraindications: This device is contraindicated for catheter insertion in the subclavian vein medial to the border of the first rib, an area which is associated with higher rates of pinch-off. The device is also contraindicated: When the presence of device related infection, bacteremia, or septicemia is known or suspected. When the patient’s body size is insufficient for the size of the implanted device. When the patient is known or is suspected to be allergic to materials contained in the device. If severe chronic obstructive lung disease exists. If the prospective insertion site has been previously irradiated. If the prospective placement site has previously suffered episodes of venous thrombosis or vascular surgical procedures. If local tissue factors will prevent proper device stabilization and/or access. Jet LT CVC Indications for Use: The Jet LT CVC Central Venous Catheter is designed for long-term central venous access. It can be used for total parenteral nutrition (TPN), infusion of I.V. liquids, blood, blood products and drugs. It can also be used for repeated withdrawal of blood samples. Contraindications: This catheter is NOT intended for any use other than that which is indicated. It is NOT recommended for use in hemodialysis or in hemoperfusion procedures, nor in patients with severe chronic obstructive lung disease.
