Jet I-Series Jet V-Series PD Accessories IMPORTANT RISK INFORMATION JET I-SERIES Indications for Use: The I-Series Peritoneal Dialysis Catheter is indicated for short-term or long-term use where peritoneal access is required for the purpose of peritoneal dialysis for Acute Kidney Injury (AKI) or Chronic Kidney Disease (CKD). Contraindications: The catheter is not intended for the Tenckhoff trocar method of insertion. The catheter is contraindicated for patients with: Infected anterior abdominal wall. Active peritonitis. Known or suspected allergies to any of the components of the catheter or the kit. JET V-SERIES Indications for Use: The V-Series Peritoneal Dialysis Catheter is indicated for short-term or long-term use where peritoneal access is required for the purpose of peritoneal dialysis for Acute Kidney Injury (AKI) or Chronic Kidney Disease (CKD). BARBED LUER LOCK ADAPTOR Indications for Use: The Jet Medical Barbed Luer Lock Adaptor is indicated for long-term use as a connection point for peritoneal dialysis catheters and male luers. Contraindications: Known or suspected allergies to the device. TITANIUM LUER ADAPTOR Indications for Use: The Jet Medical Titanium Barbed Luer Lock Adaptor is indicated for long-term use as a connection point for peritoneal dialysis catheters and male luers. Contraindications: Known or suspected allergies to the device. PD CATHETER STYLET Indications for Use: The Jet Medical Peritoneal Catheter Stylet is indicated for transient use to keep a peritoneal dialysis catheter straight and maneuverable during insertion into the peritoneum. Contraindications: Known or suspected allergies to the device. PD CATHETER TUNNELER Indications for Use: The Jet Medical Peritoneal Catheter Tunneler is indicated for transient use to dissect tissue to create the subcutaneous tunnel opening for the placement of a Jet Medical peritoneal dialysis catheter. Orders should be for full box quantities, accompanied with purchase order number. Products are shipped via United Parcel Service or Fed Ex and are F.O.B. Origin in the Continental United States. Express service is also available at the expense of the customer. Export shipments are shipped air freight to maintain traceability. Should a customer wish air parcel post shipments, Jet Medical® SA will not assume responsibility for lost shipments. All shipments outside of the Continental United States are EX Works Origin. Terms All orders will be billed at prevailing prices at the time of shipment. Terms are net 30 days for domestic accounts. Outside the U.S., terms are prepayment by wired funds or letter of credit at sight. Prices are in U.S. dollars, and are subject to tax, if any. All prices are subject to change without notice. Return Goods Policy Return goods must be authorized by Jet Medical® SA prior to return. Jet Medical® SA will accept return of any standard product within 30 days, upon approval, providing it is undamaged and in original sealed package, and returned prepaid. Custom products are nonreturnable.
